About our Medical Privacy Notice:
We are required by law to maintain the privacy of Protected Health Information and to give you this Notice explaining our privacy practices with regard to that information. You have certain rights – and we have certain legal obligations – regarding the privacy of your Protected Health Information, and this Notice also explains your rights and our obligations. We are required to abide by the terms of the current version of this Notice.
For self pay – we offer a 50% discount if paid in full at time of service and a 25% discount if on a payment plan.
We also offer flexible payment plan option for people needing to pay their balance that allows for 0% interest if paid off as part of our payment plan agreement.
Our list of accepted insurances is on the top of this page. We welcome all major insurances. Please verify with your insurance provider to confirm in-network coverage.
Patient Rights & Responsibilites
